Abstract

Type 2 diabetes mellitus is a chronic condition requiring continuous treatment, including patient adherence to prescribed medication. This study aimed to explore the relationship between medication adherence, blood glucose levels, and quality of life in patients with type 2 diabetes mellitus at Ciamis Regional Hospital, conducted from January to March 2025, the research employed a quantitative, prospective cross-sectional design involving 49 hospitalized patients. Data included blood glucose levels (RPG, FPG, 2hPG), medication adherence measured using the Medication Adherence Report Scale (MARS-5), and quality of life assessed via the Diabetes Quality of Life-Brief Clinical Inventory (DQOL-BCI). The chi-square test was used to analyze the correlation between blood glucose and medication adherence, while the Spearman rho test evaluated the relationship between medication adherence and quality of life. The study found a statistically significant but weak negative correlation between medication adherence and glycemic control (r = –0.347, p = 0.003), as well as between adherence and quality of life (p = 0.015). Interestingly, some patients with low adherence still demonstrated good glycemic control and high quality of life. These findings suggest that medication adherence alone does not fully determine treatment success or perceived well-being in patients with type 2 diabetes. Therefore, a holistic and interdisciplinary approach encompassing pharmacotherapy, education, psychosocial support, and lifestyle counselling is essential. Understanding broader factors influencing outcomes can guide the development of more effective intervention strategies in diabetes care.
