Abstract

Mental disorders constitute an economic burden on health, estimated at USD 2.5 trillion in 2010 and expected to increase to USD 6 trillion by 2030. Treatment of schizoaffective disorder involves a combination of medication and psychotherapy. Pharmacoeconomics is an important consideration in drug selection. This study aims to determine the cost-effectiveness of psychopharmaca therapy combination compared with psychopharmaca therapy and ECT combination in schizoaffective inpatients at the RSJ Provinsi Jawa Barat. Data collection was conducted retrospectively from patient medical records for the period December 2023–December 2024. Total costs, based on a health perspective, include psychopharmaca drug costs, inpatient care costs, medical procedures and support costs, ECT procedures, medical disposables costs, other drug costs, and rehabilitation costs. Effectiveness is calculated based on the Length of Stay (LOS) value. The results showed that the psychopharmaca combination group had an ACER value of IDR 603,473 and the psychopharmaca + ECT combination group had an ACER value of IDR 791,933. Meanwhile, the ICER value was IDR 1,442,887 per 1-day decrease in LOS. The psychopharmaca combination group was more cost-effective than the psychopharmaca + ECT combination group. The sensitivity test results on the ICER value between the psychopharmaca combination group and the psychopharmaca + ECT group showed that LOS value and hospitalization costs were the most influential factors on the ICERA→B value. Thus, the combination of psychopharmaca in schizoaffective patients can increase the cost-effectiveness value based on the ACER and ICER analysis results from this study.
